Skilled Nursing Services

Services provided by Professional Licensed Registered Nurse or Licensed Practical Nurse in accordance with applicable professional licensing statues and associated rules. Services includes regularly assessing the nursing need of the clients, participates in the establishment and implementation of the client’s service plan, which shall be provided in accordance with clients service plan. And also report problems and progress to supervisory personnel or the client’s personal physicians.

Personal Care Services

This refers to the provision of personal care and assistance, standby assistance, supervision or cues for person’s inability to perform one or more of the basic activities of daily living. This includes but is not limited to bathing, toileting, ambulation and transfer, grooming, shaving, dental care, dressing and eating. These services are performed by qualified Certified Nursing Assistant under the supervision of a nurse and in accordance with the clients service plan. License nurses will also be allowed to performed personal care task.

Extended Personal Support Services

This refers to those tasks designed for members who need assistance with activities of daily living, as well as relieve those person(s) normally providing care and/or oversight.

Companion/Sitter Services

This includes assisting clients with mobility, reminding (medication, dates, routines, etc.), preparing meals and feeding, running errands (prescription pick-up, laundry, etc.), grocery shopping, escorting to social events and appointments, organizing and reading mails, and household tasks essential to cleanliness and safety.
